Sholom Home Care is available wherever you live. Sholom Home Care brings you the services you need when and where you live - whether it's in one of Sholom's buildings, your own home or apartment or elsewhere in the Twin Cities.
Sholom Home Care provides client-centered care by a highly trained team of nurses, home health aides, companions and homemakers. Our goal is to help each client reach and maintain his/her highest level of function and independence. Schedules are available 24 hours a day, seven days a week.
Our registered nurse will assess your health history, confer with you and coordinate your physician to develop an appropriate plan of care to meet your individualized needs.
Your service plan may include:
- Case management services
- Skilled nurse visits
- Medication management and set-up services
- Physical, occupational and speech therapies
- Companion and homemaking services
- 24-hour and 12-hour overnight staffing
All services are available at private pay rates.
